Ahwatukee Sports & Spine has merged with Barrow Brain and Spine .

Please call our office at (480) 763-5808 to schedule an appointment or click here to register as a new patient.

We usually allow 45-60 minutes for a new patient evaluation. We do not overbook, so please give us as much notice as possible if you will be unable to make your appointment once scheduled. This will allow us to schedule someone on our waiting list.

In order to make your visit as thorough as possible, and to maximize the time spent discussing your medical condition with the Physician, we ask that you:

• Register as a new patient a fill out the medical history forms on our Portal
• Please bring a copy of your medical records (your physician may fax these to our office)
• Please bring a disk or films of any images (CT, MRI, X-Rays) that have been taken

In addition, it is your responsibility to obtain any referrals or authorizations which may be required by your insurance plan.
